This is currently just some brainstorming notes toying with the idea of creating a new (but similar) system for wounds and damage in True20. Primarily because the current system seems a bit confusing and inflexible (automatic stuns and all that).

Damage Summary

When struck with an attack make a Toughness save versus a difficulty equal to 15 plus the attackers Damage Bonus. If the target fails their Toughness save the amount they failed by are recorded as Wound Points which can effect the character's future checks and they must make a Fortitude Save with Difficulty equal to the current accumulated number of Wound Points, including those from the current attack, to avoid further effects.

Wound Points

Wound Points are the total amount the target has failed each damage save by from various attacks. Heroes suffer a -1 penalty on all checks for each 5 full Wound Points they have accumulated. Wound Point Penalties are as follows:

Wound Points(penalty): 0-4(0), 5-9(-1), 10-14(-2), 15-19(-3), 20-24(-4), ...

Additional Effects

Each time a Hero acquires Wound Points they make a Fortitude Save against a difficulty equal to their current Wound Points, penalties from Wound Points do not apply to this roll. If they fail this save by 5 or more they are Stunned for a round, if they fail by 10 or more they are unconscious (for non-lethal damage) or disabled (for lethal damage).

Lethal and Non-Lethal Damage

Lethal damage comes from most weapons, while non-lethal damage comes from things like fists, saps or the flat of a blade. The wounding and additional effects of these types of damage are generally the same other than the unconscious and disabled effects when failing a Fort Save by 10 or more. Recovery from non-lethal damage is usually considerably faster however so it's good to keep track of how many of your Wound Points are from each source.

Recovery

Working on it, Some sort of con check per time to remove 1 Wound Point with other checks to remove shaken and wake up from disabled/unconscious.
